We are seeking an exceptional individual to join our Older Persons and Abilities Support Service (OPASS) team as Care Manager Home Care Packages in a Temporary Full-Time, Fixed Term until 30 June 2026.

The Care Manager Home Care Packages primary objective is to:

Provide assistance to older people to remain living in their home in accordance with government policy. The position works in partnership with consumers, carers, families and relevant agencies to facilitate the consumer directed support and services that meet the client’s goals.

Key responsibilities include:

  • Develop and document care recipient/carer focused care plans, including budget, having regard to their preferences and vision, as well as to available resources.
  • Provide verbal and written information to all care recipients, which clearly sets out fees, budgets, service availability etc. in a Service Agreement, in accordance with the Australian Government Home Care Package Guidelines.
  • Provide ongoing case management for Home Care Package (HCP) recipients/carers, conduct regular reviews of their needs and amend care plans as appropriate.
  • Liaise and advocate with other relevant service providers regarding individual care needs and required levels/types of services.
  • Maintain up to date care recipient/carer files including Care Plans and care notes. 
  • Ensure that services are culturally relevant to the needs of care recipients/carers.
  • In conjunction with the care recipient/carer, undertake regular risk assessments and provide support to the care recipient/carer to mitigate risks.
  • Represent Council’s Older Persons’ and Ability Support Services team in case management meetings, care planning/review meetings and networks both within and outside of Council.
  • Respond in a timely manner to correspondence, requests for information and complaints.

 

About You:

To support your success in this role you will have:

  • Qualification/and or experience in Allied Health, Community Services, Nursing or Social Sciences.
  • Demonstrated knowledge of the Home Care Package Program (HCP) and Commonwealth Home Support Program (CHSP) Guidelines, the Aged Care Act 1997; the Aged care (transitional) Act 1997 and an understanding of the Victorian Health Records Act.
  • Highly developed written and verbal communication skills dealing with a range of internal and external stakeholders.
  • Current Victorian Driver’s License.
  • Current First Aid Certificate, or willingness to obtain.
  • Police Check Clearance and current Working with Children Check

Salary information: $84,218.68 to $90,918.88 per annum plus superannuation.
