About the Role:
An exciting leadership opportunity awaits an experienced Registered Nurse to step into the pivotal role of Clinical Care Manager at Gracehaven Aged Care Service in Bundaberg.
As our Care Manager, you’ll play a key role in shaping the clinical excellence and compassionate culture that defines us. Working closely with the Service Manager, you’ll mentor a dedicated team of clinicians and contribute to an environment where residents, families, and staff all thrive.
What You’ll Do
- Lead and mentor a skilled clinical team, fostering a culture of collaboration, compassion, and quality care.
- Coordinate and promote a consultative approach to resident care planning and continuous improvement.
- Ensure Aged Care Funding Instrument (ACFI) / AN-ACC data accuracy and compliance.
- Oversee clinical documentation, care planning, and external service provision.
- Monitor and review nursing practices to uphold the highest standards of resident wellbeing.
What You Bring
You’re a natural leader who combines strong clinical expertise with genuine care for people. You thrive on supporting others to achieve their best and take pride in delivering exceptional care.
- Current AHPRA registration as a Registered Nurse
- Proven ability to lead, coach, and inspire clinical teams
- Solid knowledge of aged care accreditation, compliance, and AN-ACC
- A current National Police Check or willingness to obtain.
- Australian Work Rights
Post-graduate qualifications in Geriatrics, Dementia or other Aged Care specialisation are desirable.
