LDK Seniors’ Living’s flagship village, Greenway Views, is located in Tuggeranong directly opposite South.Point Shopping Centre. About the Role 
We are currently seeking an experienced Care Partner to join our care team. LDK Care Partners work closely with our residents and/or their representatives to ensure their care needs are well supported and aligned with available funding options. This is a fantastic opportunity for a Registered Nurse looking to step into a coordination role, combining their clinical knowledge with the ability to navigate Support at Home requirements. 
Working Monday to Friday, 8:30am to 5:00pm, this role is predominantly administrative, offering a great opportunity to move away from shift work and direct clinical care, while still making a meaningful impact on resident outcomes. 

As a Care Partner, you will: 
•    Build strong relationships with residents and families  
•    Support residents to access health and community services  
•    Work with GPs, nurse practitioners and allied health to support individual care plans  
•    Liaise with rostering to ensure timely service delivery  
•    Complete documentation accurately and in line with LDK policies and standards 
 
Qualifications & Skills 
•    Current AHPRA registration as a Registered Nurse  
•    Strong communication skills with a hands-on approach  
•    Excellent time management and problem-solving skills  
•    3–5 years’ relevant nursing experience  
•    Experience in Home Care Packages, community or aged care settings (desirable)
