The Opportunity
We are looking for a dedicated Family Wellbeing Practitioner to empower Aboriginal and Torres Strait Islander families across the Lockyer and Somerset regions. In this vital role, you will deliver intensive, culturally safe case management designed to foster child safety and family resilience.
You will work closely with families and community members to assess needs and co-design holistic support plans. By providing practical advocacy and capacity-building, you will help families take the lead in creating safe, nurturing environments where children can thrive. Key Responsibilities

Deliver intensive case management, including assessments, case planning, service coordination, and regular reviews

Build strong, culturally safe relationships with children, families, and community members

Provide practical support, advocacy, and referrals to appropriate services

Collaborate with internal teams and external stakeholders to ensure coordinated care

Maintain accurate documentation, reporting, and compliance with organisational and funding requirements
What You’ll Bring
-
Strong understanding of the health and wellbeing challenges impacting Aboriginal and Torres Strait Islander peoples, including social determinants of health, intergenerational trauma, and the child protection system

Demonstrated experience working with vulnerable children, families, and communities with complex needs

Proven ability to deliver effective case management and achieve positive outcomes

Experience in applying trauma-informed, culturally safe, and strengths-based approaches

Excellent communication and relationship-building skills, with the ability to engage sensitively and effectively

Tertiary qualification in social work, psychology, human services, or a related field highly regarded
