Key Responsibilities

• Provide person-centred support in line with individual support plans
• Assist with daily living activities, including personal care (where required)
• Support participants with community access and appointments
• Encourage independence, choice, and control
• Maintain clear shift notes and follow organisational policies
• Work collaboratively with the care team and management

Essential Requirements

• Female support worker (required due to participant needs)
• Certificate III or IV in Disability, Individual Support, or similar (or willing to obtain)
• NDIS Worker Screening Check (or willingness to apply)
• Working With Vulnerable People Check (WWVP)
• Current First Aid & CPR
• Valid Australian driver’s licence and reliable vehicle
• Good communication skills and professional boundaries

Desirable

• Experience supporting participants with psychosocial or complex needs
• Experience in community access and supported accommodation
• Medication assistance experience (or willingness to train)
