Full-Time Support Coordinator - Wollongong

Salary: $70,000-$80,000 + Superannuation
Join a growing, participant‑focused NDIS provider committed to delivering high‑quality, person‑centred support across the Illawarra. We are seeking an experienced and motivated Support Coordinator to join our team full‑time in Wollongong.

About the Role
As a Support Coordinator, you will work closely with NDIS participants to help them understand their plans, build capacity, connect with the right supports, and navigate services with confidence. You will play a key role in ensuring participants receive safe, effective, and goal‑aligned support.

Key Responsibilities

  • Coordinate supports in line with participant goals, NDIS plans, and funding
  • Build strong relationships with participants, families, and service providers
  • Assist with service agreements, referrals, and linking participants to appropriate supports
  • Monitor service delivery, address risks, and ensure continuity of care
  • Maintain accurate case notes, documentation, and compliance records
  • Support participants to build independence, self‑advocacy, and capacity
  • Manage a caseload efficiently while delivering high‑quality service

About You

  • Experience in NDIS Support Coordination or case management (1-2 years preferred)
  • Strong understanding of NDIS processes, funding categories, and compliance
  • Excellent communication and stakeholder engagement skills
  • Organised, proactive, and confident managing competing priorities
  • Able to work independently and within a supportive team
  • Competent with CRM systems, documentation, and Microsoft Office

  • NDIS Worker Screening Check
  • Working With Children Check
  • National Police Check
  • Driver's licence and reliable vehicle
  • Relevant qualification in Community Services, Social Work, Psychology, or similar (preferred)
