This part time opportunity (3 days per week, days negotiable) focuses on case management and care coordination for Support at Home residents, while providing clinical care delivery to community residents as required.

A requirement to be successful for this role is a Bachelor of Nursing and current RN Registration with AHPRA

The position 

As a Case Manager (Clinical) - RN, you will play a pivotal role in supporting integrated and coordinated resident care and service requirements at The Verge Retirement Village. You'll be responsible for pre-entry assessments, ongoing clinical assessments, care planning, accessing externally funded care, service coordination, referral management and resident support.

Working collaboratively with residents, families, service providers and village team members, you'll help ensure residents receive quality services that support them to remain independent and engaged within the community.

You will be based at The Verge Retirement Village, with occasional travel to other locations as required.

Reporting to the Care Manager, your key responsibilities will include:

  • Conducting pre-entry assessments for incoming residents
  • Providing move-in support to ensure a seamless transition
  • Conduct ongoing resident assessments and develop tailored care plans
  • Coordinate aged care assessments (ACAT) via My Aged Care where required
  • Deliver clinical care to community residents as required
  • Monitor resident wellbeing and review care outcomes regularly
  • Managing referrals to external healthcare providers and services
  • Provide guidance and support to Care Service Workers
  • Build strong relationships with residents, families and stakeholders

We’re on the lookout for someone with:  

  • Current AHPRA registration as a Registered Nurse
  • Bachelor of Nursing (or Diploma equivalent)
  • Experience in aged care, home care or community nursing
  • Understanding of My Aged Care and care coordination processes
  • Strong communication and relationship-building skills
  • Current driver's licence and reliable transport
  • A current Nationally Coordinated Criminal History Check (or willingness to obtain)
  • First Aid & CPR Certificate (or ability to obtain)
  • Prepared to undertake a pre-employment medical assessment including a drug and alcohol test
